The team care model described includes physicians, nurse practitioners, physician assistants, licensed practical nurses and medical assistants, and works to advance the health and wellness of patients and of staff. The patient and family is set at the centre of the care team, which results in improved professional satisfaction and engagement among the staff as well as improved outcomes for patients and better communication and teamwork.
